End-user legal residents of the United States, District of Columbia and Puerto Rico, who are 18 years
of age or older, who purchase a Nokia Lumia 710 device during the “Offer Period” (defined below) at
participating T-Mobile® retail stores or T-Mobile dealer stores in the 50 United States, the District of
Columbia or Puerto Rico, or online at www.tmobile.com, will be eligible to receive one (1) Nokia
Xpress-On Cover (ARV: $16.99) by mail, while supplies last (the “Offer”). Limit one (1) Offer per
device/person/household. Void where prohibited. All claim submissions must include the
original sales receipt showing purchase price and store, dated between January 11,
2012, and April 30, 2012, and the 15-digit IMEI of the device purchased.  
 
THE OFFER PERIOD: The Offer is available for devices purchased between January 11, 2012, and
April 30, 2012 (the “Offer Period”).
REDEMPTION DETAILS: Complete an Online Submission Form at www.nokiacolorcover.com
(available March 1, 2012, through April 30, 2012), including  full name, mailing address (no P.O.
Boxes), city, state, ZIP code, phone number, email address, date of purchase and 15-digit IMEI of the
device purchased.  The IMEI number can be found on the UPC sticker on the device box, or under
Settings/About/More Info on your Nokia Lumia 710 system menu. Upon submission, consumer must
click “Print Form” to print out their completed Online Submission Form.
 
Once an Online Submission Form is completed, consumers must submit in one mailing envelope: (1)
the completed Online Submission Form, (2) original sales receipt showing purchase price and
store, dated between January 11, 2012, and April 30, 2012, and (3) original UPC and barcode
panel from the eligible Nokia Lumia 710 device package to: Nokia Xpress-On Cover Offer, Dept.
533800, P.O. Box 342140, Memphis, TN 38184. Claims must be postmarked by May 14, 2012, and
received by May 21, 2012.  
 
Copies of UPC, store receipts or barcodes/IMEIs that are counterfeited, mechanically reproduced or
altered in any way are void and will not be accepted. Only U.S. products are eligible for the offer. Nokia
Inc., Tic Toc and their respective subsidiaries and/or affiliates are not responsible for illegible,
incomplete, lost, damaged, postage-due, mutilated, misdirected or late submissions or mail. Unless
covered under an applicable warranty, purchased merchandise must not be returned.  Nokia reserves
the right to substitute this advertised item with an item of similar value at its sole discretion. Retailers,
retail employees, dealers, carriers and their families are not eligible for this Offer.  No group or
organization requests or purchases will be accepted. Except as otherwise permitted by law, this Offer
limited to one (1) Nokia Xpress-On Cover per eligible Nokia Lumia 710 device purchased between
January 11, 2012, and April 30, 2012, with a maximum of one (1) Nokia Xpress-On Cover per eligible
